The MI Membership Application Form is a one page summary of all that is needed to apply

The MI Members Standards Statement outlines the standards policy for our members. It includes a statement of faith as well as an appendix. After reading it, fill in the MI Membership compliance Questionnaire and return with the Application Form

The MI Members Levy explains the fee structure for membership and also needs to be filled in and returned with the above two forms. Please note that fees only start once you have been accepted and that you only pay a National fee regardless of how many state branches you may be part of!
